By submitting this Work Order Request, you acknowledge and agree to the following:

Unless covered by an active Service Agreement, service calls are subject to applicable service charges plus a $105 travel fee each way. Requested service dates and times are preferences only and are not guaranteed; dispatch priority is based on urgency, availability, location, and management discretion.

NTE Authorization: A Not-to-Exceed (NTE) amount is not an estimate or quote. It authorizes HSP Refrigeration to perform and bill for labor, diagnostics, travel, parts, materials, and other applicable service charges up to the NTE amount without additional approval. Actual charges may be less. Additional authorization will be requested before exceeding the NTE, unless otherwise provided by an applicable Service Agreement or emergency-service terms. NTE limits do not apply to Emergency Service, which is billed at applicable time-and-materials rates.

Service and diagnostic charges apply to time, travel, troubleshooting, and work performed and are not contingent upon successful repair. Charges may apply if equipment cannot be repaired, additional parts or authorization are required, replacement is recommended, or the customer declines further repairs.

By submitting this request, you confirm that you are authorized to request service for the location and will provide reasonable access to the property and equipment.
